Your First Visit
Thank you for choosing our office for your dental needs.
We strive to provide a comfortable environment where you'll feel confident in the level of dental care you receive.
When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.
For your convenience, our patient forms are available for you to download. Feel free to print and fill out the forms and bring them with you to your first appointment. These documents require the use of the free Adobe Acrobat Reader. If it's not already installed on your computer, download it here.
Most new patient visits will consist of cleaning, any necessary x-rays and a thorough examination. This visit will help us get acquainted with each other and learn about your goals and desires for your dental treatment.
If your first visit is for a dental emergency, only the tooth in question will be addressed. Depending on the diagnosis, an additional appointment with adequate time may be necessary.
We believe that good dental care begins with open communication. We promise to discuss with you any symptoms present that may require further dental treatment, our diagnosis, alternative treatments that may exist, and our recommendations. Working together, we can achieve a true partnership, with one common goal-keeping your smile beautiful and your teeth, gums and jaw joints healthy!

Payment Options
We deliver the finest care for our patients.
We are committed to providing quality and affordable dental health care. You have multiple options for payment available. For your convenience we accept cash, check, money orders, major insurance, Visa, Mastercard, Discover and American Express.
For larger treatment plans we are pleased to offer CareCredit with low or no interest payment options. Please click on the logo for more information. We will work with you to fit quality care into your budget by financial arrangements or by staging treatment over time. Like most things do not be afraid to communicate your situation.
Payment is required at the time services are performed. Other financial arrangements are available when extensive dental care is necessary, or when the dentistry performed exceeds your budget or insurance benefits. We will be sensitive to your financial circumstances within the framework of sound business practices.

Dental Insuarance
Our office is committed to helping you maximize your insurance benefits.
We accept Metlife PPO, Delta and Cigna PPO. If you have dental insurance, please provide us with verification prior to any treatment being rendered. Once we have verified your coverage, we will inform you of your co-payment for each appointment.
Please understand that dental insurance is a benefit and that you are ultimately responsible for all charges. In addition, if you have had a change in your insurance coverage, it is your responsibility to inform the front desk staff of these changes so that our records may be updated.
Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion (co-payment) must be paid at the time of treatment. As a service to our patients, we will bill insurance companies for treatment.
At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage. To do otherwise would violate our contract with you.

Our Dental Savings Plan, ideal for patients without insurance, allows you to spend less money for quality dental care. For a low, annual fee each member will receive two exams, two cleanings and routine hygiene x-rays per year along with additional benefits and discounts.

*The Dental Savings Plan is not insurance, but a discount plan. It cannot be used along with any other dental plan, workers’ comp claims, referrals to specialists, insurance claims, or in conjunction with other discounts or promotions. Coverage is in effect for one calendar year beginning on membership enrollment date with membership fees paid in full. Monthly membership fees are to be paid for a minimum 12-month period are are non-refundable. Appointment times are limited. It is the responsibility of the member to schedule appropriate visits. Any dental procedure in progress or completed before or after a member's eligibility period is excluded. Other restrictions apply.
